Claims should be submitted through the Utah Health Information Network (UHIN). Visit the website for more information.

Receiving Funds From MHC

We must have a W-9 on file before we can make any payments to you. You can fax your W-9 form to 406-447-5799. If we do not have a direct deposit set up for your group, we will send a paper check.

Newsletters

Spring 2020

The CO-OP is responding to the needs of its members during this time. We have waived all co-pays and deductibles for Coronavirus testing, should it be medically necessary for members. Its the right thing to do for them and the communities we serve.

Winter 2019

Our winter newsletter is a bit late, but still full of great information. We've got future editions scheduled so you won't miss anything important. We even have a new team member to make sure the spring edition is out soon.
